It is important that you observe the notice period of three months to the end of the contract term.

• You can cancel the contract via the Allianz website. Fill in the form, then you can print it out and send it by post.

• In addition to your name and address details, you must have your contract number to hand.

• It is best to send the completed form by registered mail to: Allianz Private Krankenversicherungs-AG, 10870 Berlin

• Alternatively, you can write the letter yourself. Here, too, your data must be included, including the contract number and date.

• In your letter, request confirmation of your termination.
